Do you have new patient paperwork?
Once you have scheduled your new patient appointment online, you will receive an email to access your online intake paperwork. Please complete all required documents for your specified appointment prior to arriving at the office for your first visit. Should you decide to fill out your paperwork in the office prior to your appointment, please arrive 15-20 minutes early.
Do you accept my insurance?
In order to provide comprehensive and integrative healthcare, our office is not currently in-network with insurance providers. This allows our patients to avoid the hassles of co-pays, deductibles, non-covered services, and visit restrictions so that you can receive top-quality care not commonly covered by insurance policies at a more reasonable price point. Our office accepts cash, check, debit/credit cards, as well as HSA/FSA cards.
Additionally, we are happy to provide a detailed invoice known as a ‘Superbill’ upon request. If your current insurance plan provides out-of-network coverage, you can submit this document to your insurance company for potential reimbursement for any covered services under your provider plan.
What can I expect at my first visit?
Initial Exam
Each new patient appointment includes a comprehensive history and physical examination to determine the extent of care required to restore health and improve overall function. Potential red flags are ruled out and any co-management opportunities with other physicians, including needs for digital imaging studies or blood labs are discussed. Detailed care plans are developed and specifically designed to fit each patient’s needs and healthcare goals. Dress comfortably and be sure to bring your water bottle with you!
Treatment
Based on your selected service, once the initial exam is complete, Dr. Charlotte will then provide treatment in the form of Chiropractic, Muscle Therapy, Acupuncture, and/or Reiki services. She will walk you through the various areas of the body as she assesses each of your concerns, including spinal mobility, cranial structure, muscle compensation patterns, meridian imbalances, and correlated organ system dysfunction. The designated treatment time can be used as an open forum for any questions you may have regarding your health and wellbeing.
Report of Findings
A report of findings will be generated once Dr. Charlotte has had thorough time to review your intake documents and objective data gathered during your initial visit and treatment session. This report will be provided during your second follow-up visit, and Dr. Charlotte will answer any additional questions you may have regarding your care plan moving forward.
Note: A Functional Health Analysis visit is focused around identifying specific lab tests, nutritional deficiencies, underlying disease causing agents, potential food sensitivities, and recommended nutraceutical supplementation. This specific visit involves an in-depth analysis utilizing Applied Kinesiology muscle testing techniques. This option is different than a “service-type” treatment visit and is designed to complement any current care plan.
